_____ Medical/Liability Release Form (THIS MUST BE NOTARIZED prior to sending to the China Call office.) (submit online, by email or print and mail)
_____ Visa Application Form. See following website to determine which Chinese embassy or consulate will process your visa: http://www.china-embassy.org/eng/hzqz/t84229.htm, Once the specific region has been identified, then you must determine a travel agent or processing company in the city of your Chinese Embassy or Consulate who will take your passport and visa application to be processed.
